At Clinical Data Solutions we know that each customer agency is unique and has its own way of managing and delivering services.  We designed TCM to be flexible so that you can continue your every day processes while at the same time benefit from the power and efficiencies of a central data management system. TCM gives you the best of both worlds.

  • Program Specific Assessments – Case managers prepare strength-based assessments online using a combination of text and user defined needs & strengths.

  • Assessment Tools – Standard and user designed assessment tools can be added to provide outcome measurement and program evaluation.

  • Historical Placements/Legal Encounters – Placements and involvement with the court system are a permanent part of the electronic record.

  • Juvenile Justice – Tracks perpetrators, victims, restitution, court placements, and more.

  • Treatment Plans – Program-specific treatment plans summarize needs, strengths, outcomes and interventions. For managed care or wrap around services, case managers can authorize services and track costs as part of the comprehensive plan of care.

  • Medications and Diagnoses - Current and historical tracking of medications at the consumer level and diagnoses at either the consumer or enrollment level as appropriate.

  • Family Member Data – Demographic and basic clinical information on the consumer’s family (whether or not they are enrolled) is stored relationally and historically.

  • Team Member Data – Demographic information on individuals associated with the consumer’s plan of care is stored relationally and historically.

  • Consumer Sequestering – Case administrators may designate a consumer as Sequestered, which restricts access to the information about that consumer to specific staff members who have been granted access.

  • Financial Histories – Consumer financial histories, including third party coverage, income, ability to pay, funding eligibility, managed care data, etc. are stored historically on all consumers.

  • Contact/Service Information – Contact data on services provided to consumers includes provider, date of service, type of service, service code or activity and other pertinent information.

  • Claims Management and Generation – Supports the latest HCFA 1500 claim forms, full-featured electronic claims and remittances functionality including third party insurance.

  • Staff and Consumer Appointment Scheduling – Appointments may be scheduled as part of a group or individually per consumer.  A full reporting system including disposition of appointments and current appointment log is available.

  • Consumer Releases of Information – Consent forms or other releases can be tracked by type and date.

  • Direct Clinical Access – Allows clinicians direct access to consumer data on a need-to-know basis.  Clinicians helped to design the graphical user interface of the client/consumer screens and so they love to use TCM.  As clinicians are the original source of data entry, this leads to rapid adoption and support of TCM in a new installation.

  • Electronic Charting – Each consumer contact record includes data for billing and statistical reporting as well as a clinical summary of the encounter. Clinicians use the system to record case notes that are centrally available for all needing access to them.

  • System Security – HIPAA compliant, role based layered security ensures that all sensitive clinical information is accessed only on a need-to-know basis.

  • Reporting – Built in and ad hoc report writing features are standard. Third party tools can also be used to access data.
